Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be carried out the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment for consultation or treatment. Please assist us at the time of your initial visit to the office by providing the following information:
- Your referral slip and x-rays from your referring dentist.
- A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website.)
- If you have dental insurance, please bring any forms or insurance cards with you to the appointment. This will save time and allow us to help you process any claims.
- A parent or guardian must be at the consultation appointment to accompany all patients under 18.
Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial joint replacements, diabetes, high blood pressure, etc.) or if you are on any medication (i.e. anticoagulant therapy, medications for treatment of osteoporosis such as bisphosphonates, etc.).
